第一,我们核对了当时的检针设备的检测记录与产品检针记录,所有的产品都有通过检针,检测设备在所有的时间段测试灵敏度都正常,产品检针有三个全检除段,一是绣花片完成后100%检针,车缝完成后100%检针,手工完成后在包装装箱前100%检针,有可能出问题的环境流程因素一一确定正常。
第二,在第一步调查中,确定我们的流程不存在问题后,我们请求贵司把详细图片拍给我们,经过展开分析发现,这种行为是一种有目有计划的蓄意报复的人为行为,因为当时我司正处在一个出货高峰期,成品仓己放不下,有一部分的成品箱放在车间及前台附近,我们对人员因素进行分检,当时有一职员消极待工,在人事主管和上级主管找他谈话后,没有改过,随后总经理亲自找他谈话,根据我们的分析,可能他觉的总经理找他谈话伤害了他的自尊,之后他就不断的到劳动局去投诉,而劳动局来调查也并非像他所说的,跟他谈过后就离开了,随后还是消极待工,影响其它职员的正常办工,最后公司无柰只好补他一个月工资跟他解除劳动关系。
第三,离职过程未按公司管理规定上交工具和利器,办理离职手续,给他把剪刀放进产品装进车间外成品箱的机会。
发生了这种事故,我们除了震惊、抱歉,向客户和消费者致以诚挚的道歉外,更多的是一种警醒和行动,为了表示我们道歉的诚意,我公司在完成以上调查结果之后,立即成立由厂长领导的整改小组,放在公司战略第一位来执行,从现场环境、场所的整改到流程的培训与检查频率、力度,以及成品的管理和出货前的安检措施进行全方面的整改,彻底的杜绝人为因素可能导致危害的出现。
